Epistaxe (sangramento nasal) é definida pela ocorrência de um escape de sangue proveniente de qualquer local da mucosa nasal. Esta mucosa (“revestimento interno do nariz”) é de fato muito frágil, formada por uma fina camada de células. Abaixo delas, há um rico sistema formado pela união de diversos vasos sanguíneos (anastomoses). Estes vasos sanguíneos fazem parte de dois dos principais sistemas que enviam sangue para face, pescoço e sistema nervoso central. Portanto, devido a esta fragilidade da mucosa e devido a estas exuberantes anastomoses, observa-se uma frequência elevada de epistaxe
Faixa etária
Ocorre uma maior frequência de epistaxes na população pediátrica. Geralmente são episódios autolimitados e com pouca repercussão para o paciente. Contudo, os episódios de epistaxe são menos frequentes nos adultos e podem ser mais graves, trazendo mais transtornos ao indivíduo
Causas
Assim, qualquer doença que promova um dano na mucosa nasal ou na parede destes vasos sanguíneos poderá levar a epistaxe. São causas de sangramento nasal em crianças: infecções locais na mucosa nasal, manipulação local com dedo, rinite, rinossinusite, uso de corticoides nasais, entre outras. São causas de sangramento nasal em adultos: associação de hipertensão arterial sistêmica + doença aterosclerótica, uso de medicações (antiagregante plaquetário e/ou anticoagulante, por exemplo), tumores nasais, uso de dispositivos nasais em pacientes internados (sondas nasais para alimentação), traumas locais, desvio de septo nasal, entre outras. Em pacientes com epistaxe de repetição, devem ser afastadas causas medicamentosas, doenças hematológicas (hemofilia, doença de von willebrand), tumores, entre outras. Por fim, em alguns indivíduos, não é possível se identificar a causa do sangramento nasal (“Epistaxe idiopática”).
Tratamento
É função do médico otorrinolaringologista avaliar o paciente para tentar identificar a causa, local do dano na mucosa nasal, repercussão do sangramento e desta maneira, propor o melhor tratamento para cada indivíduo. O tratamento pode variar desde aplicação de medicações nasais locais até cirurgia.
