Os distúrbios de detecção do olfato são classificados em hiposmia (diminuição do olfato) e anosmia (ausência total do olfato). As alteração do olfato (Hiposmia/Anosmia) podem promover um impacto negativo na qualidade de vida. Assim, estes distúrbios podem provocar transtornos de ansiedade, depressão e isolamento social nestes indivíduos. Ademais, podem apresentar diminuição do apetite e consequentemente desnutrição, pela perda deste sentido.
Hiposmia/Anosmia em idosos
Idosos com estes sintomas devem ser investigados para o início de doenças neurodegenerativas (Doença de Alzehimer, Doença de Parkinson e Doença de Lewy). Em um estudo de revisão, descreveu-se que a prevalência de distúrbios de olfato em doentes com estes diagnósticos foi de 85 – 90%. Apresentaram uma instalação precoce destes sintomas. Portanto, indivíduos com mais de 65 anos de idade com hiposmia ou anosmia devem ser obrigatoriamente investigados para estas doenças.
Causas frequentes de Hiposmia/anosmia
Além da alteração de olfato relacionada à idade, a grande maioria destes distúrbios está relacionada a três principais processos: 1-Trauma Crânio encefálico (TCE), 2- após Infecção Aguda das Vias Aéreas Superiores (IVAS), 3- Rinossinusite Crônica. As IVAS são as causas mais frequentes de perda de olfato. Contribuem com cerca de 20-30% de todas as etiologias de hiposmia/anosmia na população. Diversos vírus podem, portanto, provocar estas alterações, através de diversos mecanismos, entre eles Rhinovirus, Picornavirus (Rhinovirus/Enterovirus), Parainfluenzavirus, Coronavirus, Epstein Barr vírus.
SARS-COV2 x Hiposmia/Anosmia
Em Abril de 2020, foi publicado um estudo em que 79,7% dos pacientes com COVID-19 apresentaram hiposmia e/ou anosmia. Ademais, 56% apresentaram recuperação lenta deste sentido. Alguns casos desta doença podem se manisfestar quase que exclusivamente com este sintoma. Portanto, os indivíduos com alteração recente de olfato devem permanecer isolados por 14 dias e investigados para esta doença infecciosa.
Mecanismos de Hiposmia/Anosmia
Biopsias da região olfatória em doentes com hiposmia/anosmia pós IVAS demonstraram uma substituição do epitélio olfatório pelo respiratório. Similarmente, este pode ser o mesmo mecanismo no caso da da infecção pelo SARS-COV2. Entretanto, alguns autores sustentam que poderia ocorrer um processo inflamatório do nervo olfatório (neuropatia).
Manejo da Hiposmia/Anosmia
Cabe ao médico otorrinolaringologista avaliar o quadro clínico do pacientes com hiposmia/anomia, realizar um bom exame físico, preferencialmente com uso da endoscopia nasal, solicitar exames subsidiários (quando necessário) e indicar o tratamento adequado para cada indivíduo.
